Who is this form for?

Anybody supporting a family member or friend with a health condition who are applying for their first Carers Passport discount card.

Who is eligible?
As long as you are an unpaid carer as described above, and you have links to Hertfordshire (live, work, care, GP surgery, school), then you are eligible.
The following groups are not eligible: foster parents, care workers/paid workers, or kinship carers where the child does not have a disability or condition.

Looking to renew your card?
Depending on the contact details we have for you, we will send you a renewal form by email, text or post when your renewal is coming up. You do not need to do this form.

Lost your Carers Passport? Contact us for a replacement. You do not need to complete this form.

Please be advised that Carers Passport cards can now take up to 4 weeks to be issued and sent. We are working hard to reduce this timescale and thank you for your patience in the meantime.
