Apply for a Carers Passport

Please use this form to register for your Carers’ Passport. Please note, if you have lost your Carers’ Passport Discount Card and need a replacement you do not need to complete this form; instead, please contact us. Do not use this form to renew your card – we will post a renewal form to you the month before your card expires to check a few details.

We are currently experiencing incredibly high volumes of Carers Passport applications and in addition to the ongoing impact of COVID please be advised that Carers Passport cards can now take up to 9 weeks to be issued and sent. Please be assured we are working hard to reduce this timescale as quickly as we can, although in the meantime thank you for your patience.

Please note: The following groups are not eligible for the Carers in Hertfordshire passport card: Foster parents, care workers/paid workers, kinship carers where the child does not have a disability or condition.
