0-25 Together and Preparing for Adulthood

If so please come along to one of our talks from the 0 – 25 Service Manager, who will be talking about the support they provide for young people and their carers.

    Please note: this meeting is for Parents/Carers of Children and Young People (aged 0-25 years) with additional needs.

    Would you like to have the opportunity to learn more about the Hertfordshire County Council’s 0 – 25 Together service?  Would you like to have your voice heard about your experience with the service?

    If so please come along to one of our talks from the 0 – 25 Service Manager, who will be talking about the support they provide for young people and their carers.

    As well as general information about 0 – 25 Together as a service, we will be looking at:

    • The process of a Child & Family Assessment
    • Information about what you can expect after an Assessment has been completed.
    • The approach to Carers Assessments
    • Information about Short Breaks including Direct Payments and Home Care
    • Information about How you can access 0-25 Together.
    • How 0-25 supports young people preparing for adulthood (PFA)

     There will be an opportunity to ask questions and share the issues you face as a carer.
