Accessing our services is quick and easy. You can register yourself as a carer with us or a GP, nurse, health care worker, or other professional can make a referral.
Carers in Hertfordshire is a charity that provides free services and support to unpaid carers – people looking after a relative, friend or someone close to them, due to illness, disability, old age, or substance misuse.
We support children and adults in Hertfordshire that have a caring role or those who live elsewhere and look after someone in the county. We also support bereaved carers with similar links to Hertfordshire for three years after the death of the person they cared for.
Our services to unpaid family and friend carers include information and advice over the phone and online, one to one and group support, and workshops and events. If you would like to speak to one of our Carer Support Advisors about a caring role you or someone you are referring has please call 01992 58 69 69 (office hours Monday to Friday).
Registering yourself for our support (self-referral)
Referrals from GPs, nurses and other healthcare professionals
If you would like to refer someone to our services please complete the Carer Identification and Professional Referral Form.
If you are referring someone to our services, please ensure that they have given explicit consent for the referral and are aware that it is happening. Please also make sure that you have completed all sections of the form.
Referrals from professionals other than GPs and NHS staff
If you are referring someone and you are from a local authority, a volunteer or community sector organisation, pharmacy, care home, or are a Social Worker (services outside of GP practices and hospitals), please complete the Professionals Carer Referral Form.
Please note that if you are referring someone other than yourself consent must be obtained before making a referral and the person must not lack mental capacity.
