Use this form to register for your first Carers' Passport.

Do not use this form to renew your card. We will post a renewal form to you the month before your card expires to check a few details.

If you have lost your Carers’ Passport Discount Card and need a replacement, please contact us. You do not need to complete this form.

We are currently experiencing incredibly high volumes of Carers Passport applications. Please be advised that Carers Passport cards can now take up to 9 weeks to be issued and sent. We are working hard to reduce this timescale and thank you for your patience in the meantime.

The following groups are not eligible for the Carers in Hertfordshire passport card: Foster parents, care workers/paid workers, kinship carers where the child does not have a disability or condition.
