Supported Discharge from Adult Mental Health Services

Tuesday 18th November 2025, 10.30am - 12.30pm Improving how individuals are supported when leaving adult community mental health services. We understand that discharge can sometimes feel unclear or unsettling. This project aims to co-produce a new model that ensures people feel informed, involved, and supported throughout the transition. We are exploring ideas such as personalised discharge planning, step-down support through voluntary and community sector partners, and options for reconnecting with services if needed. Your experiences—whether as a service user or carer—are essential to shaping this work. We would greatly value your input on what discharge currently feels like, what could be improved, and how we can build a future model that better meets people’s needs.
